Post Telephone Interview Requirements
Following our telephone interview with you and preliminary acceptance of your application into our program we will require the following:
- Evidence of having successfully completed a substance abuse treatment program or long term transitional living program, (and permission to contact).
- Letters of recommendation from people who are familiar with you, your addiction and your recovery experiences.
- A substance abuse professional or counselor.
- A personal reference.
- A letter from you on your own behalf helping us to get to know you, specifically
- Why do you want to enter into our program.
- What have been your biggest challenges in sobriety.
- How long you’ve been clean and sober.
- How you see yourself being a contribution in this community and a statement of your goals.
- What you hope to achieve.
